Create an Intake Form
Intake forms let you collect information from prospective clients before opening a case. You build them as templates in Clasp and can reuse them across different case types.

The Intake page lists your templates and the entry point to create one.
Create a new intake form template
- Click Intake in the sidebar.
- Click + New Template in the top-right corner.
- Enter a name for the template (for example, "General Intake" or "Family Law Intake").
- Add fields to collect the information you need. See Add Fields to an Intake Form for details.
- Save the template.
Managing your templates
Your saved templates appear on the Templates tab of the Intake page. Click any template to edit it.
Tip: Create separate templates for different practice areas — for example, one for family law and one for immigration — so each form only asks the questions relevant to that case type.
